Gardens and Jacobs Centre
Stellar Healthcare provide GP Primary Care Services to the patients at Gardens and Jacobs.
Gardens and Jacobs Neurological centre provides specialist inpatient rehabilitation and complex disability management services for men and women who have complex neurological conditions including acquired brain injury, spinal injury as well as neuroprogressive conditions.
The purpose-built facilities at Gardens and Jacobs include bedrooms fitted with high-dependency equipment including ceiling track hoists, wall mounted suction and oxygen equipment and rehabilitation therapy spaces. All rooms are adapted to individual requirements.
Gardens and Jacobs offers a full interdisciplinary team to support and enable residents to undergo reviews of their specific and/or changing needs and work towards targeted and monitored goals. As a care centre this service is a home from home offering individual bedroom accommodation and spacious communal areas with dedicated therapy spaces, a gym, gardens, and our own transport.
The service is supported by a team of dedicated professionals including specialist Consultants in Rehabilitation, GPs, nurses, and therapists.
Gardens and Jacobs offers a range of care and rehabilitation pathways to suit the individual needs of those accessing our specialist services. These include:
- Neurological physical and cognitive rehabilitation, inpatient assessment and treatment
- Longer term complex care including specialist nursing, respiratory, tracheostomy and ventilator care
- Sensory assessment and complex care for patients with disorders of consciousness
- Effective management of behavior’s that challenge
- Respite and short breaks
- Palliative care
Visiting Specialists
Both Units are integrated within the service provision of their local health communities. Visiting specialists are engaged, after being previously familiar with the needs of individual residents, across the following key areas: spinal, respiratory, spasticity, ENT, continence, bladder and bowel, seating, psychology, enteral feeding, tissue viability, plus many other specialists brought in as required to meet the needs of our residents.
